About the Role

We are seeking a highly skilled and compassionate Care Coordinator Specialist to join our team at Integral Care. As a key member of our interdisciplinary team, you will play a vital role in providing psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses.

Key Responsibilities
  • Develop and implement individualized treatment plans to support clients in achieving their desired life outcomes.
  • Provide assessment and evaluation of clients' strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorders.
  • Collaborate with external and internal providers to ensure optimal and personalized services.
  • Deliver therapeutic interventions to foster skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need.
  • Facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities.
Requirements
  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field.
  • Current and valid driver's license and a clean driving record.
  • Bilingual preferred.
