Job Summary

The Care Coordinator/Specialist provides psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions, utilizing a recovery-based and trauma-informed perspective.

Care Coordinators/Specialists operate in a dynamic environment, developing extensive knowledge of community resources, collaborating with interdisciplinary teams, and cultivating a clinical skill-set by forging therapeutic relationships with diverse community members.

Interventions and services are typically provided in the community, requiring the use of a personal automobile and cell phone.

Key Responsibilities
  • Deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need.
  • Facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities.
  • Participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services.
  • Coordinate services with external and internal providers to support individuals in achieving independent functioning.
Requirements
  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or a related field.
  • Current and valid driver's license and a clean driving record.
