Job Summary

We are seeking a highly organized and detail-oriented Medical Care Coordinator to join our team at Peoples Community Clinic. As a key member of our healthcare team, you will play a vital role in ensuring the efficient and effective coordination of patient care with outside entities, including medical specialists and diagnostic facilities.

Responsibilities
  • Coordinate Patient Care: Verify patient insurance, obtain authorizations and referrals for diagnostic exams and specialist visits, and ensure continuity of care.
  • Effective Communication: Maintain open communication with patients and clinical staff regarding upcoming appointments, follow-ups, and results.
  • Electronic Health Records: Maintain accurate and up-to-date electronic health records, ensuring referrals are closed and diagnostic results are entered daily.
  • Task Management: Manage task boxes and use reporting systems to track follow-ups and successful outcomes.
  • Preventative Care: Participate in adult and women's health outreach and follow-up programs, including mammography, PAP, and colposcopy screenings.
  • Patient Outcomes: Maintain and manage pregnancy outcomes, complete WIC renewal forms, and identify opportunities for process improvements.
  • Quality Assurance: Adhere to HIPAA guidelines and identify opportunities to improve workflow, work processes, and patient care.
Qualifications
  • Education: High school diploma or equivalent required.
  • Experience: One year of experience in a medical office or customer service role.
  • Skills: Bilingual in English and Spanish, excellent customer service skills, sharp attention to detail, strong ability to multitask, and good verbal and written communication skills.
  • Certifications: Certified Medical Assistant (CMA) preferred.
