Job Summary

The Care Coordinator/Specialist provides psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions. This role operates in a dynamic environment, developing extensive knowledge of community resources and collaborating with interdisciplinary teams to cultivate a clinical skill-set.

Care Coordinators/Specialists work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. They participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services.

Care Coordinators/Specialists may provide therapeutic interventions, foster skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need.

This role requires a flexible work schedule and may have additional duties associated with providing bilingual communication services. Regular, predictable attendance is considered an essential requirement for this position.

Responsibilities
  • Provide psychosocial rehabilitative skills training to individuals with severe mental illnesses

Requirements
  • Graduated from an accredited college or university with a Bachelor's degree in Human/Behavioral/Social Services or related field
  • Current and valid driver's license and a clean driving record
  • Bilingual preferred
