Job Summary: We are seeking a highly skilled Care Coordinator/Specialist to provide psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses. The ideal candidate will have a strong understanding of community resources and be able to collaborate with interdisciplinary teams to cultivate a clinical skill-set.

Key Responsibilities:

  • Provide psychosocial rehabilitative skills training to individuals with severe mental illnesses
  • Develop and maintain knowledge of community resources
  • Collaborate with interdisciplinary teams to provide comprehensive care
  • Cultivate a clinical skill-set through therapeutic relationships with diverse community members

Requirements:

  • Graduated from an accredited college or university with a Bachelor's degree in Human/Behavioral/Social Services or related field
  • Current and valid driver's license and a clean driving record
  • Bilingual preferred
