Position Overview

SUMMARY

The role involves delivering comprehensive case management support to individuals participating in a program aimed at assisting homeless populations. This includes conducting initial assessments and ongoing evaluations, alongside regular follow-ups and referrals to community resources. The ultimate objective is to facilitate client independence and secure stable housing.

KEY RESPONSIBILITIES

  • Ensure compliance with established policies and protocols for programs assisting homeless individuals, including guidelines related to meals, curfews, attendance at meetings, and transportation to appointments.
  • Design a consistent program schedule that encompasses activities, meetings, and meals, while leveraging external resources such as guest speakers.
  • Verify that all participants fulfill HUD entry requirements and that necessary documentation is accurately maintained in client files.
  • Conduct initial assessments of service needs for all clients entering the program and develop an Individual Service Plan within three days of their entry.
  • Collaborate closely with Shelter staff, Relapse Prevention Managers, and Sober Housing personnel to identify potential program participants.
  • Maintain regular contact with clients actively engaged in the program.
  • Establish and uphold a consistent contact schedule with all participants for a minimum duration of 24 months.
  • Keep detailed files and case notes for each participant.
  • Ensure accurate statistical data is collected and reported on a monthly basis.
  • Engage with treatment programs and other community resources to create referral networks, maintaining a detailed log of all referrals made.
  • Provide crisis intervention services, including 24-hour cell phone availability.
  • Engage clients by understanding and addressing their needs, whether they fall within or outside the scope of the role.
  • Participate in engagement team meetings as required and maintain effective communication with clients and staff across various departments.
  • Uphold confidentiality regarding client, employee, and agency information in accordance with applicable federal and state laws and funding requirements.
  • Ensure adherence to program, department, agency, and funder requirements, as well as organizational policies and procedures.
  • Perform additional duties as assigned.

QUALIFICATIONS AND SKILLS

  • Bachelor's Degree in a relevant field, or a minimum of 5 years of experience working with the target population may be considered.
  • Possession of safe and reliable transportation.
  • Experience in substance abuse support is required.
  • Strong interpersonal and organizational skills are essential.
  • Previous experience in group facilitation is preferred.

ORGANIZATIONAL STRUCTURE

  • This position reports directly to the Coordinated Assessment Supervisor.
  • There are no direct reports associated with this role.

PHYSICAL DEMANDS

  • Ability to operate a vehicle.
  • Capability to navigate stairs.

WORK ENVIRONMENT

Travel within the state is a requirement of this position. The case manager will have direct or incidental contact with clients served by the organization across various programs funded or administered through relevant state offices. A successful background check is mandatory.
