POSITION SUMMARY
Provide comprehensive case management services to participants in a program dedicated to assisting chronically homeless individuals with histories of relapse. This role involves initial and ongoing assessments, regular follow-ups, and referrals to community-based services. Responsibilities include conducting home visits and facilitating group sessions.
KEY RESPONSIBILITIES
- Ensure compliance with policies and protocols for programs supporting chronically homeless individuals, including guidelines for meals, curfews, meeting attendance, and client transportation to appointments.
- Develop and maintain a regular program schedule that includes activities, meetings, and meals, while accessing external resources as needed.
- Verify that all participants meet HUD requirements for program entry and that all necessary documentation is present in client files.
- Conduct initial service needs assessments for all clients entering the program and create Individual Service Plans within three days of entry.
- Collaborate closely with Shelter staff, Relapse Prevention Managers, and Sober Housing staff to identify new program participants.
- Maintain daily contact with clients actively engaged in the program.
- Establish and uphold a regular contact schedule with all participants for a minimum of 24 months.
- Keep accurate files and case notes for each participant.
- Ensure precise statistical data is maintained and reported monthly.
- Conduct outreach to treatment programs and other community resources to establish referral contacts, maintaining a detailed log of all referrals made.
- Provide crisis intervention services, including 24-hour cell phone coverage.
- Engage all clients by understanding and addressing their needs, whether within or outside the scope of work.
- Attend and participate in engagement team meetings as requested, ensuring effective communication with clients and staff across various areas.
- Uphold confidentiality of client, employee, and agency information in accordance with federal and state laws and funder requirements.
- Ensure adherence to program, department, agency, and funder requirements, as well as SMOC policies and procedures.
- Perform other duties as assigned.
QUALIFICATIONS
- Bachelor’s Degree in a related field, or a minimum of 5 years of experience working with the target population.
- Experience in substance abuse is required.
- Strong interpersonal and organizational skills are essential.
- Previous experience in group facilitation is preferred.
ORGANIZATIONAL STRUCTURE
- Reports directly to the Program Manager, CSPECH/PFS.
- This position has no direct reports.
PHYSICAL REQUIREMENTS
- Ability to drive a vehicle.
- Ability to ascend and descend stairs.
WORKING CONDITIONS
Travel within the state is required. As part of this role, the Case Manager will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is mandatory.
