POSITION SUMMARY

The Triage Case Management Specialist will deliver comprehensive case management services to individuals experiencing housing instability at the Emergency Triage Center. The primary focus will be on assessing service needs and collaborating with clients to formulate an Individual Self-sufficiency Plan aimed at promoting self-reliance and facilitating a transition to stable housing.

KEY RESPONSIBILITIES

  • Conduct thorough case management assessments for individuals facing housing crises seeking assistance at the center.
  • Complete all necessary documentation and gather required information to ensure an accurate emergency housing and service needs assessment.
  • Adhere to contracts, organizational policies, and relevant regulations.
  • Act as a liaison between clients and other social service agencies, housing staff, or municipal representatives.
  • Document initial service needs for each client, focusing on critical areas such as Health, Housing, Substance Abuse, Mental Health, Employment, Legal Issues, Benefits, and Domestic Violence.
  • Provide clients with initial program orientation to support their journey towards self-sufficiency and independent living.
  • Maintain confidentiality, establish boundaries, and ensure compliance with program rules and regulations.
  • Keep accurate and timely case notes and update service plans as necessary.
  • Assist clients in scheduling appointments and arranging transportation as needed.
  • Engage with clients to understand and address their needs, whether within or outside the scope of the role.
  • Participate in team meetings and maintain effective communication with clients and staff across various departments.
  • Protect the confidentiality of client, employee, and agency information in accordance with applicable laws and funding requirements.
  • Ensure compliance with program, agency, and funder requirements, as well as organizational policies and procedures.
  • Perform additional duties as assigned.

QUALIFICATIONS AND SKILLS

  • Bachelor's Degree or a minimum of three years of experience in Human Services or a related field; CADAC certification is preferred.
  • Demonstrated sensitivity to the needs of low-income individuals from diverse backgrounds.
  • Ability to collaborate effectively as part of a dynamic team.
  • Strong assessment, advocacy, and case management skills.
  • Reliable transportation and a valid driver's license with the ability to meet insurance requirements.
  • Bi-lingual candidates are preferred.

REPORTING STRUCTURE

  • This position reports directly to the Triage Services Manager.
  • There are no direct reports for this position.

PHYSICAL DEMANDS

  • Must be able to drive.

WORK ENVIRONMENT

Travel to various locations may be required. The Triage Case Management Specialist will have direct or incidental contact with clients served by the organization in various programs funded or administered through relevant health and human services offices. A successful background check is mandatory.
