We are seeking a highly skilled HIV Nurse Case Manager to join our team at Edward M. Kennedy Community Health Center, Inc. in Worcester. As a key member of our healthcare team, you will provide comprehensive care to patients with HIV, ensuring they receive optimal treatment and support.
Key Responsibilities- Coordinate Patient Care: Manage the care of HIV patients, ensuring they receive evidence-based treatment, education, and preventive care across multiple health settings.
- Document Patient Progress: Accurately document patient care, progress, referrals, and services in the Electronic Health Record (EHR) in accordance with health center and nursing standards.
- Assess Patient Needs: Conduct regular patient assessments to identify medical and social needs, including barriers to care.
- Develop Individual Service Plans: Create and monitor customized Individual Service Plans (ISPs) with patients to ensure they receive tailored care and support.
- Provide Comprehensive Services: Offer a wide range of services as part of a primary care team, linking patients to appropriate services that promote positive health outcomes, treatment adherence, and self-sufficiency.
- Meet Reporting Requirements: Maintain accurate and timely documentation, meeting all data submission requirements to funders using E2 Boston, and reporting HIV and AIDS cases to the Massachusetts Department of Public Health (MDPH) State Lab.
- Collaborate with the HIV Team: Participate in regular case conferences with the HIV Team and represent the team at community events, trainings, and meetings.
- Ensure Quality and Compliance: Adhere to quality, compliance, and regulatory standards, performing other duties as assigned.
- Registered Nurse License: Possess a current Registered Nurse License in Massachusetts.
- CPR Certification: Hold current CPR certification.
