POSITION SUMMARY

The Triage Case Management Specialist is responsible for delivering comprehensive case management services to individuals experiencing housing instability at the Emergency Triage Center. This role involves conducting thorough assessments of service needs and collaborating with clients to create personalized Self-sufficiency Plans aimed at achieving greater independence and stable housing solutions.

KEY RESPONSIBILITIES

  • Conduct intensive case management assessments for individuals facing housing crises seeking assistance at the center.
  • Complete all necessary documentation and gather required information to ensure accurate emergency housing and service needs evaluations.
  • Ensure adherence to contractual obligations, organizational policies, and relevant regulations.
  • Act as a liaison between clients and other social service agencies, housing staff, or municipal representatives.
  • Document initial service needs across critical areas: Health, Housing, Substance Abuse, Mental Health, Employment, Legal, Benefits, and Domestic Violence.
  • Provide initial orientation to clients, facilitating their transition towards enhanced self-sufficiency and independent living.
  • Maintain confidentiality and enforce program rules, including the good neighbor policy.
  • Keep precise and timely case notes and update service plans regularly.
  • Assist clients in scheduling appointments and arranging transportation as needed.
  • Engage with clients to understand and address their needs, both within and outside the scope of the role.
  • Participate in team meetings and maintain effective communication with clients and colleagues.
  • Safeguard the confidentiality of client, employee, and agency information in compliance with applicable laws and funding requirements.
  • Ensure compliance with program, agency, and funder standards, as well as organizational policies.
  • Perform other duties as assigned.

QUALIFICATIONS AND SKILLS

  • Bachelor's Degree or a minimum of three years of experience in Human Services or a related field; CADAC certification is preferred.
  • Demonstrated sensitivity to the needs of low-income individuals from diverse backgrounds.
  • Ability to collaborate effectively as part of a dynamic team.
  • Strong assessment, advocacy, and case management skills.
  • Possession of reliable transportation, a valid Massachusetts driver's license, and the ability to meet insurance requirements.
  • Bi-lingual candidates are preferred.

ORGANIZATIONAL STRUCTURE

  • This position reports directly to the Triage Services Manager.
  • There are no direct reports for this role.

PHYSICAL REQUIREMENTS

  • Ability to drive is essential.

WORKING ENVIRONMENT

This role requires travel to various locations within Massachusetts. The Triage Case Management Specialist will have direct or incidental contact with clients served by the organization across various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is a prerequisite for this position.
