SUMMARY

The role involves delivering comprehensive case management services to individuals residing at the Emergency Triage Center. The primary focus is to assess service needs and collaborate with clients to create a personalized Self-sufficiency Plan aimed at achieving greater independence and stable housing solutions.

PRIMARY RESPONSIBILITIES

  • Conduct thorough case management assessments for individuals facing housing crises seeking assistance.
  • Complete all necessary documentation to ensure an accurate emergency housing and service needs evaluation.
  • Adhere to compliance standards outlined in contracts, company policies, and other governing documents.
  • Act as a liaison with other social service providers, housing staff, and municipal officials.
  • Document initial service needs across key areas: Health, Housing, Substance Abuse, Mental Health, Employment, Legal, Benefits, and Domestic Violence.
  • Provide initial program orientation to clients, facilitating their transition towards self-sufficiency and independent living.
  • Maintain confidentiality and enforce program rules, including the good neighbor policy.
  • Keep accurate and timely case notes and updated service plans.
  • Assist clients in scheduling appointments and arranging transportation as needed.
  • Engage clients effectively, addressing their needs within and beyond the scope of the role.
  • Participate in engagement team meetings and maintain effective communication with clients and staff.
  • Uphold confidentiality of client, employee, and agency information in accordance with applicable laws and regulations.
  • Ensure compliance with program, agency, and funder requirements, as well as organizational policies.
  • Perform other duties as assigned.

KNOWLEDGE AND SKILL REQUIREMENTS

  • A Bachelor's Degree or a minimum of three years of experience in Human Services or a related field; CADAC certification is preferred.
  • Demonstrated sensitivity to the needs of low-income individuals from diverse backgrounds.
  • Ability to collaborate effectively as part of a dynamic team.
  • Proficient in assessment, advocacy, and case management skills.
  • Reliable transportation and a valid Massachusetts driver's license, meeting insurance requirements.
  • Bilingual skills are preferred.

ORGANIZATIONAL RELATIONSHIP

  • Reports directly to the Triage Services Manager.
  • This position does not have direct reports.

PHYSICAL REQUIREMENTS

  • Ability to drive is required.

WORKING CONDITIONS

Travel to various locations may be necessary. The Triage Case Manager will interact with clients in various programs funded or administered through relevant health and human services offices. A successful background check is mandatory.
