Position Overview:
As a Medical Social Worker, you will play a crucial role in providing support to families and caregivers during challenging times. Your primary responsibility will be to conduct initial bereavement assessments and assist in creating tailored bereavement care plans. You will help patients and their families navigate psychosocial challenges that may affect their health and access to medical services.

Key Responsibilities:
In this role, you will be expected to:

  • Facilitate effective communication with the Hospice Admissions Team and other relevant departments regarding referrals.
  • Conduct interviews with family members to evaluate their psychosocial needs and circumstances.
  • Develop a bereavement component of the family care plan within 24 hours of assessment.
  • Ensure compliance with the standards set by Hospice of Marion County and the broader healthcare profession.
  • Maintain confidentiality of patient and family information in accordance with professional guidelines.
  • Stay informed about Medicare and Medicaid policies related to admissions and skilled nursing facilities.
  • Identify and address the social service needs of families, setting achievable goals to meet those needs.
  • Assess the financial status of bereaved families and assist them in finding financial resources.
  • Collaborate closely with healthcare professionals to enhance the understanding of social and emotional factors affecting grief.
  • Provide individual or group support to help families cope with loss and grief.
  • Utilize community resources to aid bereaved individuals in reintegrating into their daily lives.
  • Prepare comprehensive histories, assessments, service plans, and reports accurately and promptly.
  • Conduct professional evaluations and provide appropriate interventions, including crisis management.
  • Respect diverse lifestyles and values while working with families.
  • Facilitate bereavement support groups to ensure positive outcomes for both the hospice and community.
  • Participate in on-call rotations to provide support as needed.
  • Deliver educational sessions related to social services and bereavement to staff and community members.

Qualifications:
To be successful in this position, candidates should possess:

  • A Master's Degree in Social Work (MSW) from an accredited institution.
  • A valid Licensed Clinical Social Worker certification in the State of Florida.
  • A minimum of three years of experience in social work, preferably in a hospice setting.
  • Strong communication skills and the ability to work independently in the field.
  • Proficiency in using office software and technology.

Work Environment:
This position requires regular mobility and the ability to lift and assist patients when necessary. You will be expected to work in various settings, including homes, hospitals, and care facilities.
