Position Overview:
The Clinical Social Services Coordinator plays a vital role in assisting patients with intricate psychosocial challenges. This position involves helping individuals navigate eligibility for social programs, funding sources, and community assistance from various support programs and agencies. Additionally, you will provide support during transitions of care and discharge planning.
This role includes offering crisis intervention to patients and families facing psychosocial difficulties and coordinating the creation of discharge plans for high-risk patient groups.
You will receive referrals from interdisciplinary team members, including physicians, RN Care Managers, and nursing staff.
Key Contributions:
• Proficient understanding of discharge planning, utilization management, care management, and performance improvement
• Knowledge of state and federal regulations relevant to Care Management
• Ability to identify suitable community resources and collaborate effectively with patients, families, multidisciplinary teams, and community organizations to achieve optimal patient outcomes
• Familiarity with pre-acute and post-acute care settings and community resources
• Maintains up-to-date knowledge of local services available, especially for patients with limited financial resources
• Strong skills in interviewing, assessment, and organization
Qualifications:
• Master’s degree in social work (MSW)
• A minimum of three (3) years of experience in hospital or medical social work
• Experience in Care Management and discharge planning
