Position Overview:
As a vital member of the East Team, the Medical Social Worker plays a crucial role in providing compassionate support to families and caregivers during their bereavement journey. This position is responsible for conducting initial assessments and developing tailored bereavement care plans that address the unique psychosocial needs of patients and their families.

Key Responsibilities:
The following duties are essential to the role and may include additional responsibilities as assigned:

  • Facilitates effective communication with the Hospice Admissions Team, Volunteer Department, and primary care staff regarding referrals.
  • Conducts interviews with family members and significant others to evaluate their situations and identify psychosocial needs.
  • Creates a bereavement component of the family care plan within 24 hours of assessment.
  • Ensures compliance with the standards and guidelines set forth by Empath Health and the professional community.
  • Maintains confidentiality of patient and family information in accordance with agency policies and professional ethics.
  • Stays informed about Medicare and Medicaid regulations related to admissions and qualifications for skilled nursing facilities.
  • Identifies social service needs and establishes goals to address those needs effectively.
  • Assesses the financial situation of bereaved families and assists in locating financial support when necessary.
  • Collaborates closely with healthcare professionals to enhance understanding of the social and emotional factors affecting the bereaved.
  • Provides individual or group support to help families navigate their grief and loss.
  • Utilizes community resources to aid bereaved individuals in reintegrating into their communities.
  • Prepares thorough and accurate documentation, including histories, assessments, and service plans.
  • Conducts professional evaluations and intervenes appropriately, including crisis intervention when required.
  • Demonstrates respect for diverse lifestyles without imposing personal values on families.
  • Facilitates bereavement support groups to foster positive outcomes for both Hospice and community organizations.
  • Completes initial bereavement assessments and provides pre-bereavement counseling as needed.
  • Participates in on-call rotations to provide coverage as necessary.
  • Delivers basic counseling services for bereaved families, up to six sessions.
  • Conducts staff and community training sessions related to social services and bereavement education.

Qualifications:
To excel in this role, candidates must meet the following qualifications:

  • Master's Degree in Social Work (MSW) from an accredited institution.
  • Licensed Clinical Social Worker (LCSW) in the State of Florida.
  • A minimum of three years of experience in social work, with hospice experience preferred.
  • Valid Florida driver's license and reliable transportation.
  • Ability to work independently in the field, exercising sound judgment.
  • Strong analytical skills to interpret various documents and regulations.
  • Proficient in writing reports and correspondence.
  • Effective public speaking skills for group presentations.
  • Knowledge of medical terminology and its application.
  • Problem-solving abilities in diverse situations.
  • Proficient in using office equipment and software, including MS Word, Outlook, and Excel.

Physical Demands:
The physical requirements for this position include:

  • Regularly sitting, walking, and using hands for various tasks.
  • Occasionally lifting and moving up to 25 pounds.
  • Ability to access various facilities, including homes and healthcare settings.
  • Effective communication with clients and colleagues.
  • Frequent mobility within and between locations.
  • Specific vision abilities, including close and distance vision.
