Position Overview:
As a vital member of the East Team, the Medical Social Worker is responsible for conducting comprehensive initial assessments for families experiencing bereavement. This role involves collaborating with families to create a tailored Bereavement Plan of Care, addressing psychosocial challenges that may hinder optimal healthcare outcomes.

Key Responsibilities:
In this role, you will:

  • Ensure seamless communication with the Hospice Admissions Team, Volunteer Department, and primary care staff regarding referrals.
  • Conduct interviews with family members and significant others to evaluate their psychosocial needs.
  • Develop the bereavement component of the family care plan within 24 hours post-assessment.
  • Uphold the standards and guidelines set forth by Empath Health and the healthcare profession.
  • Maintain confidentiality of patient and family information in compliance with agency and professional standards.
  • Stay informed about Medicare and Medicaid regulations related to admissions and skilled nursing facilities.
  • Identify and address the social service needs of families, setting achievable goals.
  • Review financial circumstances of bereaved families to assist in finding necessary support.
  • Collaborate closely with healthcare professionals to enhance the understanding of social and emotional factors impacting grief.
  • Facilitate individual or group support to help families navigate their loss.
  • Utilize community resources to aid bereaved individuals in reintegrating into their communities.
  • Prepare thorough and timely documentation, including histories, assessments, and service plans.
  • Conduct professional evaluations and provide appropriate interventions, including crisis management.
  • Exhibit sensitivity to diverse lifestyles while maintaining professional boundaries.
  • Lead bereavement support groups to foster positive outcomes for both Hospice and community organizations.
  • Participate in on-call rotations to provide support as needed.

Qualifications:
To be successful in this position, candidates must possess:

  • A Master's Degree in Social Work (MSW) from an accredited institution.
  • A valid Licensed Clinical Social Worker certification in the State of Florida.
  • A minimum of three years of social work experience, preferably in a hospice setting.
  • Strong communication skills and the ability to work independently in various environments.
  • Knowledge of medical terminology and the ability to interpret regulations and procedures.

Physical Requirements:
This position requires the ability to perform essential functions, including lifting and moving patients, and frequent mobility in various settings.
