Position: Director of Social Services
Employment Type: Full Time
Sign-On Bonus: $2,000
Experience in long-term care is preferred.
Role Overview:
As the Director of Social Services, you will oversee our Social Services Department and its initiatives. Your leadership will be vital in promoting the emotional, social, and psychological health of our residents.
Key Responsibilities:
- Design and execute comprehensive social services programs tailored to meet the varied needs of our residents.
- Provide guidance to the Social Services team, ensuring the delivery of exceptional, person-centered care.
- Collaborate with interdisciplinary teams to develop personalized care plans that enhance residents' overall well-being.
- Serve as a liaison among residents, families, and community resources to facilitate smooth transitions into our facility and assist with discharge planning.
- Advocate for residents' rights, ensuring they receive the highest quality of care while maintaining their dignity and independence.

Qualifications:
Essential Requirements for the Director of Social Services:
- Master’s degree in social work or a related discipline.
- Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) preferred.
- A minimum of 4 years of experience in social services, with at least 2 years in a leadership role within a long-term care environment.
- Strong knowledge of the aging process, psychosocial needs of the elderly, and the effects of chronic illness.
- Proven ability to work collaboratively with a diverse team and build positive relationships with residents and their families.
