Position Overview:
We are looking for a dedicated and skilled Acute Care Nurse Practitioner to become a vital part of our esteemed transplant program. This role is essential in delivering comprehensive care to patients throughout their transplant journey, from initial evaluations to long-term follow-up care. The successful candidate will collaborate closely with a multidisciplinary team of renowned surgeons, physicians, and nursing staff in a supportive and dynamic environment.

Key Responsibilities:
  • Perform detailed pre-transplant assessments, including thorough medical history evaluations, physical examinations, and necessary diagnostic tests to determine transplant eligibility and identify potential risks.
  • Work in conjunction with the transplant team to formulate personalized care strategies for transplant candidates, utilizing evidence-based practices and considering patient preferences.
  • Educate patients and their families about the transplant process, addressing potential risks, benefits, and post-operative care requirements.
  • Coordinate the scheduling of evaluations, tests, and consultations to ensure a timely pre-transplant workup.
  • Provide support during transplant surgeries, including intraoperative assistance and post-operative care in critical care settings.
  • Oversee post-transplant patient management, including medication administration, wound care, and monitoring for any complications to enhance patient outcomes.
  • Collaborate with primary care providers and specialists to ensure comprehensive care for transplant patients, addressing both transplant-related and concurrent health issues.
  • Engage in quality improvement initiatives, research activities, and interdisciplinary discussions to elevate patient care standards.
  • Maintain precise and timely documentation of patient interactions and care plans in electronic health records, adhering to regulatory and institutional guidelines.
  • Commit to ongoing professional development through participation in educational opportunities and relevant professional organizations.
Qualifications:
  • Master's degree in Nursing from an accredited institution with a focus on Acute Care Nurse Practitioner.
  • Active licensure as a Registered Nurse and Nurse Practitioner in the relevant state.
  • Certification as an Acute Care Nurse Practitioner from a recognized certifying body.
  • A minimum of two years of clinical experience as a Nurse Practitioner in an acute care environment, with a preference for experience in transplant or critical care.
  • Strong clinical assessment abilities and critical thinking skills to manage complex medical situations.
  • Exceptional communication and interpersonal skills for effective collaboration with diverse healthcare teams and patient engagement.
  • Knowledge of transplant medicine, including evaluation processes, surgical methods, immunosuppressive treatments, and post-operative care protocols.
  • Ability to adapt in a fast-paced work environment, demonstrating resilience and flexibility.
  • Dedication to patient-centered care, emphasizing compassion and respect for patient dignity.
  • Willingness to accommodate flexible scheduling, including on-call duties and weekend coverage as necessary.
