Position Overview:
We are in search of a skilled and empathetic Acute Care Nurse Practitioner (ACNP) to enhance our inpatient specialty Cardiothoracic Transplant team. The ideal candidate will possess a robust background in acute care environments, cardiology, thoracic surgery, or transplant medicine, along with a dedication to providing exceptional care to critically ill patients.
Key Responsibilities:
- Deliver thorough care for patients in the cardiothoracic transplant unit, encompassing assessment, diagnosis, and management of both acute and chronic conditions.
- Collaborate with a multidisciplinary team to formulate and execute personalized care plans for patients undergoing cardiothoracic procedures.
- Conduct advanced clinical procedures within the scope of practice, including central line placements, intubations, and other invasive interventions.
- Monitor patient progress, interpret diagnostic tests, and modify treatment plans as necessary.
- Educate patients and their families regarding transplant processes, post-operative care, and lifestyle changes to facilitate recovery.
- Engage in daily rounds, team meetings, and case discussions to evaluate patient care and treatment strategies.
- Maintain precise and timely documentation in electronic health records (EHR).
Qualifications:
- Master's degree in Nursing with a focus on Acute Care Nurse Practitioner program from an accredited institution.
- Current state licensure as a Nurse Practitioner with Acute Care certification.
- Prior experience in acute care, cardiology, thoracic surgery, or transplant medicine is preferred.
- Strong clinical assessment abilities and the capacity to make critical decisions in high-pressure situations.
- Excellent communication and interpersonal skills, with a commitment to patient-centered care.
