Job Summary

The University of Kansas Health System is seeking a skilled Ambulatory RN Care Coordinator to join our dynamic rheumatology clinic team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and ensuring seamless coordination of services.

Key Responsibilities
  • Manage patient flow, triage, and direct patient care to optimize patient outcomes
  • Liaise with front desk staff, nursing staff, physicians, and clinic manager to ensure effective communication and coordination
  • Develop and implement patient care plans, defining appropriate resource utilization and working with providers to set patient goals
  • Provide nursing support and coordination to optimize patient outcomes and improve quality of life through clinical and educational programs
  • Support patient self-management of disease and coordinate continuity of care with patients and families
  • Manage high-risk patient care, including patients with multiple comorbidities or high risk for readmission
Requirements
  • High School Graduate
  • Bachelor Degree in Nursing
  • 2 or more years of RN experience
  • Basic Life Support Heartsaver (BLS HS) certification
  • Licensed Registered Nurse (LRN) in good standing
Preferred Qualifications
  • Previous medical office experience
  • Current active multi-state Registered Nurse (RN) license
