Key Responsibilities- Manage patient flow, triage, and direct patient care to optimize patient outcomes and improve quality of life.
- Liaise with front desk staff, nursing staff, physicians, and clinic manager to ensure effective communication and coordination of care.
- Develop and implement patient care plans, defining appropriate resource utilization and working with providers to set patient goals.
- Provide nursing support and coordination to the practice to optimize patient outcomes and improve patient quality of life through clinical and educational programs and/or support services.
- Support patient self-management of disease and coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
- Manage high-risk patient care, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting.
- Develop relationships with patients and competently use assessment skills to identify physical, psychosocial, financial, and environmental healthcare needs of the patient.
- Educate patients and ancillary personnel about healthcare needs, safety issues, and benefits, including patient education.
- Utilize electronic tools, such as EMR, to efficiently document assessments, clinical findings, plans of treatment, and progress toward goals.
- High School Graduate
- Bachelor Degree in Nursing
- 2 or more years of RN experience
- Basic Life Support Heartsaver (BLS HS) certification from the American Heart Association
- Licensed Registered Nurse (LRN) in good standing in the state of employment
- Previous medical office experience
- Current active multi-state Registered Nurse (RN) license in good standing
