Position Title: Clinical Nurse Coordinator - Radiation Oncology

Position Summary:

The Clinical Nurse Coordinator in the Ambulatory Clinic is a licensed registered nurse who serves as a clinical authority, delivering ethical, evidence-based, safe, and compassionate nursing care.

This role emphasizes patient safety and the quality of nursing services by identifying and addressing patient needs, executing clinic-related patient care, conducting health education, advocating for patients, coordinating healthcare services, and assessing patient outcomes.

The RN Care Coordinator will assist assigned physicians by effectively communicating with patients, families, and caregivers, ensuring that vital information is relayed to the physician.

Support and nursing care will be provided during physician clinic sessions, ensuring that patients and their families understand the care plan.

Verification of all orders placed during clinic sessions will be conducted, ensuring that physicians are informed of results.

This position requires flexibility to float to various clinics as needed.


Key Responsibilities:
  • Manage patient flow, triage, and direct patient care.
  • Act as a communication liaison among front desk staff, nursing personnel, physicians, and clinic management.
  • Oversee patient care by establishing expected outcomes, defining resource utilization, and collaborating with providers to set patient goals.
  • Provide nursing support and coordination to enhance patient outcomes and improve quality of life through clinical and educational initiatives.
  • Facilitate patient self-management of health conditions.
  • Coordinate continuity of care for patients and families following hospital admissions, discharges, and emergency visits.
  • Manage high-risk patient care, including those with multiple comorbidities or at risk for hospital readmission.
  • Develop rapport with patients and utilize assessment skills to identify their physical, psychosocial, financial, and environmental healthcare needs.
  • Educate patients and relevant personnel about healthcare needs, safety issues, and benefits, including patient education.
  • Utilize electronic tools, such as EMR, for efficient documentation of assessments, clinical findings, treatment plans, and progress toward goals.
  • Observe, document, and report patient conditions and responses to medications and treatments to physicians.
  • Educate patients and families regarding diagnostic procedures, medications, nutrition, and health maintenance.
  • Maintain and review patient records, charts, and other relevant information.
  • Respond to and refer incoming patient treatment-related inquiries.
  • Instruct patients and families on medication and treatment protocols.
  • Ensure patient confidentiality is upheld.
  • Document patient care using EMR to maintain compliance with practice standards.
  • Float to different clinics as requested.

Required Education and Experience:

  • High School Graduate
  • Bachelor's Degree in Nursing (BSN required within 5 years for existing employees in the role)

Preferred Education and Experience:

  • 2 or more years of RN experience
  • Previous experience in a medical office setting

Required Licensure and Certification:

  • Licensed Registered Nurse (LRN) - Multi-State
  • State Board of Nursing - Must possess a current active multistate RN license in good standing in the state of residence or an active single state RN license in good standing in the state of employment.

Basic Cardiac Life Support (BLS or BCLS): American Heart Association (AHA)
