Position Overview:
The Bilingual Medical Case Manager plays a crucial role in delivering comprehensive medical case management services to clients living with HIV and facing additional health challenges, such as substance use disorders and PTSD. The primary aim is to enhance the overall health and quality of life for these individuals.

Key Responsibilities:
  • Conduct thorough intake assessments to gather detailed biopsychosocial histories of clients.
  • Administer client screenings to evaluate substance use, behavioral risks, and overall mental and physical health.
  • Provide immediate crisis intervention services, including suicide prevention strategies.
  • Assist clients in altering detrimental behaviors by fostering a deeper understanding of their personal challenges.
  • Facilitate individual counseling sessions focused on behavioral health, substance use, and risk mitigation.
  • Establish connections to necessary treatment and support services, ensuring effective follow-up on referrals.
  • Monitor the progress of individualized care plans and client adherence to treatment objectives.
  • Lead psychoeducational group sessions addressing topics such as substance use, coping strategies, and relationship skills.
  • Maintain accurate progress notes and essential client documentation.
  • Comply with professional standards set by regulatory bodies, ethical guidelines, and organizational policies.
  • Occasionally conduct home visits and accompany clients to medical appointments.
  • Engage in outreach initiatives and participate in significant organizational events.
  • Perform additional duties as assigned by the supervisor or Executive Director.

Qualifications:
  • A Master's or Bachelor's degree in social work, counseling, psychology, or a related field, along with three years of experience in social services.
  • Preferred certifications: CADC, LPC, LCPC, LSW, LCSW, or MISA.
  • Strong knowledge of HIV care and related support services is highly desirable.
  • Bilingual proficiency in English and Spanish is required.
  • Demonstrated cultural sensitivity towards African American, Latino, and LGBT communities.
  • Certification as an HIV/AIDS Tester must be obtained within the first six months of employment.
  • Proficient in word processing and computer applications.
  • Strong organizational and time management skills.
  • Effective presentation and group facilitation abilities.
  • Access to reliable transportation.

Job Type: Full-time

Compensation: $40,000 - $55,000 annually
