Key Responsibilities:

Under the guidance of the Director, the Community Connections Manager will cultivate and strengthen partnerships with local businesses and community organizations to create opportunities for individuals with disabilities who are engaged in prevocational training. The role will emphasize discovery activities such as business tours, volunteer opportunities, job shadowing, job trials, and other experiential learning initiatives.

Essential Duties:

  • Assist individuals in the community as they participate in training and experiences focused on Self Advocacy, Career Counseling, Work-Based Learning, Volunteerism, and Workplace Readiness.
  • Maintain accurate records and daily reports of client progress.
  • Adhere to all safety protocols when accompanying individuals into the community.
  • Collaborate with individuals to identify and explore their vocational interests and relay this information to the Community Connections Manager.
  • Maintain certifications in medication administration, CPR, and First Aid.
  • Participate in departmental meetings and training sessions.
  • Stay informed on each individual's Person-Centered Individual Support Plan (PCISP), risk assessments, and behavior plans.
  • Fulfill attendance requirements as per program, Benchmark policies, and client needs.
  • Report any suspected criminal activity involving ICF/MR residents to the appropriate authorities.
  • Immediately report any suspected abuse, neglect, or exploitation to a supervisor or department head.
  • Comply with all health and safety standards to ensure the well-being of all individuals served.

Qualifications:

  • Experience working with adults with disabilities is preferred.
  • A High School diploma or equivalent is required.
  • A valid driver's license and proof of auto insurance are necessary.
