KEY RESPONSIBILITIES:

Under the guidance of the Director, the Community Connections Manager will cultivate and strengthen partnerships with local businesses and community organizations to create inclusive opportunities for individuals with disabilities engaged in prevocational training. This role will emphasize experiential learning through activities such as business tours, volunteer opportunities, job shadowing, and job trials.

PRIMARY DUTIES:

  • Assist individuals in community engagement while participating in training focused on Self Advocacy, Career Counseling, Work-Based Learning, Volunteerism, and Workplace Readiness.
  • Maintain accurate records and daily progress reports for clients.
  • Adhere to safety protocols when accompanying individuals into the community.
  • Collaborate with individuals to identify vocational interests and relay this information to the Community Connections Manager.
  • Ensure certification in medication administration, CPR, and First Aid is up to date.
  • Participate in departmental meetings and training sessions.
  • Stay informed on each individual's Person-Centered Individual Support Plan (PCISP), risk assessments, and behavior management plans.
  • Maintain consistent attendance as required by the program and client needs.
  • Report any suspected incidents of abuse or neglect to the appropriate authorities.
  • Comply with all health and safety standards to protect the well-being of individuals served.

SKILLS AND QUALIFICATIONS:

  • Ability to work independently with minimal supervision.
  • Strong communication skills for effective interaction with staff, clients, and external stakeholders.
  • Basic computer proficiency is essential.

EDUCATION AND EXPERIENCE:

  • Preferred experience working with adults with disabilities.
  • A High School diploma or equivalent is required.
  • A valid driver's license and proof of auto insurance are necessary.
