Your Responsibilities:
As a Mental Health Case Manager, you will provide direct, hands-on support to a diverse range of individuals, fostering genuine, therapeutic, and trusting relationships while being sensitive to their unique experiences and traumas.

In this role, you will be responsible for completing necessary paperwork and documentation, as well as providing referrals and case coordination for individuals receiving various services, including mental health, medical, financial, social, educational, vocational, housing, and legal assistance.

Your duties will be performed in accordance with Rule 79, encompassing documentation, functional assessments, crisis plans, releases, and individualized community support plans, all conducted in a timely manner and per performance standards.

You will interact with clients frequently, at least once a month or as needs arise.
Additionally, you may develop relationships with county and state staff, family members, employers, and other agencies to support those receiving our services.


Over time, this position will allow you to grow and enhance your personal skill set while contributing to the development of this expanding division within our organization.

Qualifications:
To be considered for this opportunity, you should possess a BA or BS in human services or a related field; a BSW must hold a valid and current social work license.
Additionally, you will need a valid driver's license, adequate insurance coverage, and access to a vehicle.
