Position Overview:

The role involves working with a specific population to ensure effective collaboration with the healthcare team, facilitating the delivery of suitable and cost-efficient services that align with the care plan, promote independence, and enhance overall quality of life.



Key Responsibilities:
  • Conduct problem identification, psychosocial evaluations, financial counseling, and referrals to community resources.
  • Manage and coordinate a designated caseload in accordance with established guidelines, performing comprehensive psychosocial assessments to understand patient goals, support systems, resources, health status, and treatment responses.
  • Collaborate closely with nursing staff and other healthcare professionals to create and oversee a care plan aimed at optimizing patient functioning and quality of life.
  • Identify and advocate for necessary organizational and community resources to implement the care plan effectively and efficiently.
  • Maintain accurate documentation of case management activities in compliance with organizational standards.
  • Analyze patient outcomes to identify areas for improvement in program quality and cost-effectiveness.
  • Assist patients in navigating local, state, and federally funded programs and refer cases of suspected abuse or neglect to appropriate agencies.
  • Provide educational support and counseling to patients, families, caregivers, and healthcare team members, addressing issues such as end-of-life care and Advanced Directives.
  • Stay informed about current case management standards and innovations in patient care, as well as relevant laws and regulations governing social work practice.

Qualifications:
  • Experience: Minimum of three years in clinical practice and two years in case management required. At least one year of experience with the specified population is necessary.
  • Education: A Master's degree in Social Work (MSW) is required.
  • Licensure: Credentials from multiple states are necessary, with primary state credentials required before hire. Additional state credentials must be obtained post-hire.
  • Licensure as a Clinical Social Worker in Maryland, Virginia, and the District of Columbia within six months of hire, along with a Case Manager Certificate within 36 months of hire.

Preferred Skills:
  • Proficiency in computer software within a Windows environment.
  • Familiarity with community systems and resources in the service area.
  • Understanding of regulatory issues pertinent to the Mid-Atlantic region.
