Role Objectives

The Supportive Services Coordinator is instrumental in overseeing programs that promote life skills, coping mechanisms, goal achievement, and housing stability for participants. This role also involves providing linkage and referral services to enhance residents' housing stability.

Key Duties and Responsibilities
  • Deliver supportive services to address challenges and obstacles affecting progress towards individual goals.
  • Maintain tenant rosters that accurately reflect entry and exit data for relevant properties.
  • Identify early warning signs of crises and respond effectively.
  • Ensure precise record-keeping, documentation, and reporting as required.
  • Track resident engagement in services and monitor their progress towards objectives.
  • Create monthly newsletters detailing available social and supportive services, both on-site and off-site, along with a schedule of these services.
  • Compile information on off-site services and programs, providing referrals as appropriate.
  • Offer advocacy, on-site case management services, and connect residents to necessary services.
  • Conduct intake assessments and orientations for new residents.
  • Establish positive relationships and coordinate services among community supports, service providers, and property management.
  • Encourage a welcoming atmosphere for residents and promote community participation.
  • Facilitate daily Adult Education workshops for residents.
  • Plan and execute Health and Wellness events and community activities annually.
  • Input data into the Homeless Management Information System (HMIS) promptly and accurately.
  • Provide on-site life skills training and support to residents.
  • Perform additional duties as assigned.
Qualifications
  • Must be at least 18 years of age.
  • Experience working with individuals facing chronic homelessness and mental health challenges.
  • Familiarity with Housing First, Harm Reduction, and/or Recovery Oriented service models.
  • Ability to maintain professional relationships with consumers while setting appropriate boundaries.
  • Capacity to engage with culturally diverse communities.
  • Preferred AA degree or equivalent experience in assisting homeless individuals.
  • Strong team player with the ability to work independently.
  • Knowledge of independent living skills such as cooking, budgeting, and community etiquette.
  • Valid California driver's license and access to an insured vehicle.
  • Willingness to work some evenings and weekends and travel within California.
Physical Requirements

The Supportive Services Coordinator must be able to communicate effectively through speaking and listening. Tasks may include sitting, using hands and fingers for handling, standing, walking, reaching, climbing, balancing, twisting, leaning, and moving between locations. The role may also involve stooping, kneeling, crouching, or crawling. Vision capabilities should encompass both close and distant ranges. Computer operation and lifting files and reports may be necessary. Reasonable accommodations can be provided for individuals with disabilities. Ability to lift 25 lbs and automobile travel up to 10% may be required.

Work Environment

This position involves working with the homeless population in various settings, which may include streets, shelters, or other habitation areas. Potential exposures may include bed bugs, odors, and unclean environments due to homelessness or mental health factors. Some tasks may be performed in recreational or communal spaces. Noise levels are typically moderate but may vary based on activities.
