Role Objectives

The Supportive Services Coordinator is integral in overseeing programs that foster life skills, coping mechanisms, goal achievement, and housing stability for participants. This role also involves connecting residents with resources to enhance their housing security.

Key Duties
  • Deliver supportive services to address challenges and obstacles that impede progress towards personal goals.
  • Maintain tenant records that accurately reflect entry and exit data for relevant properties.
  • Identify signs of distress and crises, responding with appropriate interventions.
  • Ensure accurate documentation and reporting as required.
  • Track resident engagement in services and assess progress towards established goals.
  • Create monthly newsletters detailing available social and supportive services, both on-site and off-site, along with their schedules.
  • Monitor off-site services and program information, providing referrals as necessary.
  • Offer advocacy and on-site case management services, linking residents to required resources.
  • Conduct intake assessments and orientations for new residents.
  • Build strong relationships and coordinate services among community resources, service providers, and property management.
  • Encourage a welcoming atmosphere for residents and promote community participation.
  • Facilitate daily Adult Education workshops for residents.
  • Plan and execute annual Health and Wellness events and community activities.
  • Timely input of data into the Homeless Management Information System (HMIS).
  • Provide on-site training in life skills to residents.
  • Perform additional duties as assigned.
Qualifications
  • Must be at least 18 years old.
  • Experience working with individuals facing chronic homelessness and mental health challenges.
  • Familiarity with Housing First, Harm Reduction, and/or Recovery Oriented service models.
  • Able to maintain professional boundaries while building relationships with clients.
  • Ability to engage effectively with culturally diverse communities.
  • Preferred: AA degree or equivalent experience in serving homeless populations.
  • Strong team player with the ability to work independently.
  • Knowledge of independent living skills, including cooking, budgeting, and community etiquette.
  • Valid California driver's license and access to an insured vehicle required.
  • Willingness to work evenings and weekends, with travel within California as needed.
Physical Requirements

The Supportive Services Coordinator must possess effective communication skills, both verbal and listening. Responsibilities may include sitting, using hands for tasks, standing, walking, reaching, climbing, balancing, twisting, and moving between locations. The role may also require stooping, kneeling, crouching, or crawling. Vision capabilities should encompass both near and far distances. Computer operation and the ability to lift files and reports may be necessary. Reasonable accommodations can be made for individuals with disabilities. The ability to lift up to 25 lbs and occasional automobile travel may be required.

Work Environment

This position involves working with the homeless population in various settings, which may include shelters or other living environments. Potential exposures may include unsanitary conditions and various environmental factors related to homelessness or mental health issues. Tasks may be performed in communal spaces, with moderate noise levels that may vary based on activities.
