Role Summary

The Family Services Coordinator (FSC) operates under the guidance of the Program Supervisor and is integral to the collaborative efforts within the Families and Communities Together (FaCT) initiative. The FSC is tasked with delivering thorough case management and family support services to clients who may have encountered domestic challenges. Responsibilities include assessing client needs, facilitating access to necessary services, conducting follow-ups, administering evaluation tools, performing home visits when required, engaging in community outreach, collaborating with internal and external partners in social services, and preparing necessary documentation and reports as mandated by funding requirements.

Key Responsibilities
  • Facilitate family support by connecting families with both internal and external community resources to ensure appropriate service access.
  • Maintain accurate documentation and assessments, ensuring timely entry into relevant databases.
  • Conduct home visits in collaboration with social workers, as permitted by supervisory guidelines.
  • Adapt work hours to participate in community outreach initiatives.
  • Lead four cycles (10 weeks each) of the Personal Empowerment Program (PEP) annually at the Family Resource Center.
Required Qualifications
  • Bachelor's Degree in behavioral sciences or a related field, or equivalent professional experience.
  • Proficiency in both English and Spanish is essential.
  • A minimum of two years of experience in case management with vulnerable children and families.
  • Familiarity with the child welfare system is preferred.
  • Strong understanding of community resources and the ability to leverage these connections within the local area.
  • Possession of a valid Class C driver's license.
  • Completion of a 40-hour Domestic Violence Advocate certification training is required post-hire.
