Position Summary:

The PrEP/OEND Navigator will play a crucial role in implementing and expanding a low-threshold HIV Pre-Exposure Prophylaxis (PrEP) Program specifically designed for homeless individuals who inject drugs and other high-risk populations. This position involves providing intensive care coordination, patient navigation, and managing the PrEP patient database to support quality improvement initiatives and panel management. Additionally, the Navigator will deliver overdose prevention education and naloxone distribution to at-risk populations.

Key Responsibilities:

  • Conduct outreach in diverse environments (such as streets, shelters, and treatment facilities) to educate potential patients about PrEP, identify eligible candidates, and facilitate their access to PrEP services, including initial assessments and follow-up visits.
  • Assist patients in overcoming insurance barriers, provide transportation referrals, and proactively remind them about appointments and necessary lab work; visit patients in their residences to help them reconnect with care and provide medical case management.
  • Offer adherence counseling, coordinate medication refills with pharmacy staff, and assist with medication delivery while collaborating with partner agencies for medication storage.
  • Help clients identify health-related social service needs and facilitate access to relevant resources.
  • Provide health promotion, behavioral risk reduction, and communicable disease prevention education through health education materials and group sessions in various outreach settings.
  • Document each patient interaction in the electronic medical record and maintain accurate statistics of work performed, assisting in report preparation.
  • Engage in research activities related to the PrEP program.
  • Collaborate with partner agencies to enhance access to PrEP services and provide educational materials.
  • Administer HIV counseling and testing, including phlebotomy for various serologies and link patients with positive results to appropriate clinical services.

Additionally, the Navigator will provide overdose prevention education in outreach and off-site settings, distributing naloxone while adhering to data record-keeping and reporting requirements.

Qualifications:

  • Bachelor's Degree in social services or a related field is preferred.
  • Bilingual proficiency in Spanish is strongly preferred.
  • Phlebotomy skills must be demonstrated within 60 days of training, with additional compensation available upon proficiency.
  • Understanding of harm reduction strategies and knowledge of HIV prevention methods.
  • Experience with database management is essential.

Compensation and Benefits:

  • Starting compensation is $22.07 per hour, with increases based on experience.
  • Full-time employees are eligible for a competitive time-off policy, including 4 weeks of vacation, health, dental, and vision insurance, a 403B retirement savings plan with employer contributions, and a pre-tax MBTA pass program offering a 40% discount. Eligible employees may also receive yearly increases and additional compensation for demonstrated bilingual proficiency.
