Position Overview:

The PrEP/OEND Navigator (PN) will play a crucial role in the implementation and expansion of a low-threshold HIV Pre-Exposure Prophylaxis (PrEP) Program tailored for homeless individuals who inject drugs and other high-risk populations. The PN will deliver intensive care coordination, patient navigation, and database management for PrEP patients, contributing to quality improvement initiatives and panel management. Additionally, the PN will provide overdose prevention education and naloxone distribution to at-risk populations.

Key Responsibilities:

  • Conduct outreach in diverse environments (e.g., streets, shelters, treatment facilities) to educate potential candidates about PrEP, assess eligibility, and facilitate access to PrEP services, including coordinating laboratory testing and follow-up visits.
  • Assist patients in overcoming insurance barriers, provide transportation referrals, and proactively remind them of appointments and necessary lab work; visit patients in their residences to reconnect them with care and provide medical case management.
  • Offer adherence counseling, coordinate medication refills with pharmacy staff, and assist with medication delivery, collaborating with partner agencies on medication storage.
  • Identify clients' health-related social service needs, help them access relevant resources, and support their effective utilization of these services.
  • Deliver health promotion, behavioral risk reduction, and communicable disease prevention education through health educational groups and promotional materials in various outreach settings.
  • Document each patient interaction in the electronic medical record, maintain accurate statistics of work performed, and assist in report preparation, including managing HCV team tracking documents.
  • Engage in research activities related to PrEP program initiatives.
  • Collaborate with partner agencies to enhance access to PrEP services and provide educational materials.
  • Conduct HIV counseling and testing, including phlebotomy for HIV, HCV, and syphilis serologies, as well as PrEP intake laboratory testing; link patients with positive results to appropriate clinical services.

Additionally, the PN will provide overdose prevention education in outreach and off-site settings, either individually or in group sessions, and distribute naloxone in compliance with OEND data record-keeping and reporting requirements.

Qualifications:

  • Bachelor's Degree in social services or a related field is preferred.
  • Bilingual proficiency in Spanish is strongly preferred.
  • Phlebotomy skills must be demonstrated within 60 days following onsite training, with an additional 3% compensation added to the hourly rate upon proficiency.
  • Understanding of Harm Reduction strategies and knowledge of HIV prevention methods are essential.
  • Experience with database management is required.

Compensation and Benefits:

  • Starting compensation is $22.07 per hour, with increases based on experience.
  • Full-time employees are eligible for a competitive time-off policy, including 4 weeks of vacation, health, dental, and vision insurance, a 403B retirement savings plan with employer contributions, and a pre-tax MBTA pass program offering a 40% discount. Eligible employees may also receive yearly increases and an additional $7,500 added to the base hiring rate for demonstrated bilingual proficiency.
