We are seeking a highly skilled and compassionate RN Case Manager to join our dynamic HIV primary care team at Boston Health Care for the Homeless Program. As an HIV Nurse Case Manager, you will play a critical role in providing high-quality nursing care to patients with HIV, while also coordinating care and addressing the unique needs of individuals experiencing homelessness.
Key Responsibilities- Conduct comprehensive nursing assessments and triage for HIV team patients, providing medical advice and guidance to the care team as needed.
- Implement patient-centered plans of care, evaluating outcomes and communicating regularly with patients and care team members to ensure seamless care coordination.
- Administer vaccines, injectable medications, and provide wound care as indicated, while also providing medication adherence assessments and counseling.
- Coordinate care with integrated HIV team behavioral health staff, visiting patients at shelters and outreach sites to provide nursing care and care coordination services.
- Identify and address structural barriers to medical care and medication adherence, working with the interdisciplinary team to provide solutions and support.
- Assist medical providers in coordinating follow-up care related to medication management for opioid use disorder and other substance use disorders.
- Promote a compassionate and therapeutic environment, demonstrating initiative and commitment to improving services and processes that positively impact patient care and organizational goals.
- Licensed as a Registered Nurse in the Commonwealth of Massachusetts.
- Bachelor's degree in nursing preferred; 2 years of experience working in an ambulatory/outpatient care environment required. HIV primary care experience strongly preferred.
- Certification as AIDS Certified Registered Nurse (ACRN) preferred; however, this can be obtained after being hired, and program funding available to support this training.
- Expertise in substance use disorder and harm reduction principles, strongly preferred.
- Experience working with people experiencing homelessness strongly preferred.
- Mental Health experience, including knowledge of trauma-informed care, a plus.
- Experience working in a community health center setting, a plus.
- Compensation starting at $35.22 per hour and is commensurate with years of experience.
