Patient Services Coordinator

Join the Mount Sinai Health System as a Patient Services Coordinator - Mental Health Specialist and play a crucial role in delivering mental healthcare services to patients. As a liaison between patient care activities and the WTC Health Program, you will ensure quality patient care during visits and help educate patients on their care treatment plans.
Responsibilities:- Work collaboratively with internal and external WTC Mental Health Providers to facilitate patients' care plans by coordinating appointments and information.
- Collaborate with clinical staff to resolve issues impacting patients' care progression.
- Provide front-end coverage as needed, recording telephone encounters for routing to appropriate Providers.
- Monitor and update schedules and databases with visit status, reminders, follow-up appointments, authorizations, and resolving billing errors.
- Work as a liaison with MH Providers, patients, and clinical staff to connect patients with relevant internal/external resources.
- Provide recommendations for process/service improvement to promote patient satisfaction and address issues.
- Review, investigate, route, and follow up on patient questionnaires to assist clinical staff and administration in monitoring patients' responses.
- Document patient concerns and actions taken, including demographic data, incident synopsis, actions taken, and outcome.
- Identify systems-related problems and work collaboratively with mental health providers, administrators, and staff to resolve them.
- Conduct monthly accountability reports and assist in preparing annual and quarterly executive reports for senior leadership.
- Some college; Bachelor's degree preferred.
- Previous experience in healthcare/customer service/patient navigation or relevant experience in mental health.
- Excellent verbal and written communication skills to communicate effectively with patients, staff, visitors, and vendors.
- Ability to speak Spanish preferred.
