Job Title: Patient Services Coordinator I-Cardiology

The Mount Sinai Health System is seeking a highly skilled Patient Services Coordinator to join our team in Cardiology. As a key member of our healthcare team, you will play a vital role in delivering exceptional patient care and ensuring a seamless experience for our patients.

Responsibilities:
  • Collaborate with Primary Care Providers to facilitate patient care plans, participating in daily huddles and monthly case discussions.
  • Coordinate appointments, information, and continuing care requirements to ensure timely and effective patient care.
  • Monitor and update dashboards with reminders and alerts to organize daily tasks, including visit status reminders, follow-up appointments, and authorization for medical review.
  • Contact insurance carriers for pre-certification and consult with Department/Practice Manager to address patient needs or escalated issues.
  • Provide front-end coverage as needed, recording telephone encounters for routing to appropriate Providers.
  • Work as a liaison with Physicians, patients, and clinical staff to connect patients with relevant internal/external resources.
  • Recommend process/service improvements to promote patient satisfaction and address issues.
Qualifications:
  • Some college; Bachelor's degree preferred.
  • 3 years of healthcare/customer service or relevant experience.
