The Patient Experience Coordinator plays a vital role in improving the patient/family experience while on hospice service. This role involves providing supportive calls to patients and families at end of life, maintaining patient experience data, and communicating patient/family needs and concerns to the teams and Quality Review Coordinator.
- Manage coordination of calls initiated by workflow and primary members of the IDT, noting the need to follow up with phone calls and visits.
- Participate and contribute to the Interdisciplinary Team Process.
- Display professionalism during all communication with patients, families, and co-workers.
- Report all Patient Experience Program shift occurrences and complaints.
- High School Diploma or equivalent required
- Prior experience working within a healthcare environment, hospice preferred, knowledge of hospice HIS requirements
- Excellent verbal and written communication skills
- Mandarin/Cantonese required
- Use of various computer programs, including Microsoft Word, Excel, and PowerPoint
Salary: Min. $38,545.53/Yr., Max. $48,181.92/Yr.
