Patient Care Coordinator

We are seeking a dedicated and compassionate Patient Navigator to join our team at Apicha Community Health Centers. As a Patient Navigator, you will play a vital role in providing field-based case management services to patients enrolled in our HIV care coordination program.
Your primary responsibility will be to build strong relationships with HIV-positive patients, helping them connect and stay engaged in prevention and treatment services. You will work closely with the care team, including doctors, nurses, and other staff, to ensure optimal healthcare outcomes for our patients.
Key responsibilities include:
- Providing direct services to patients living with HIV/AIDS under the supervision of a Care Coordinator
- Carrying out tasks necessary to execute medical and support service plans, including accompanying patients to appointments, providing coaching, and delivering health education encounters
- Maintaining client-related records and other required documentation according to our protocols and standards
- Representing the agency at various venues to promote our services
Qualifications include an Associate's degree in human services or social work, at least 12 months of experience in patient navigation or a similar field, and strong socio-cultural identification with our target population. Fluency in one or more Asian languages is preferred, and excellent written and verbal communication skills in English are required.
