Essential Qualifications

  • Oversee all facets of a client's care from initial assessment to final discharge.
  • Facilitate cohesive collaboration among team members towards shared objectives.
  • Communicate and evaluate client health information within the team framework.
  • Identify eligible patients and connect them with necessary services.
  • Review treatment plans and coordinate caregivers efficiently.
  • Engage with justice system stakeholders to ensure optimal outcomes for clients.
  • Champion patients' rights and advocate for appropriate care.

Role Description

Become a part of our team in a position that entails delivering high-quality case management interventions to prospective clients and supporting individuals in treatment facilities. Your duties will encompass assessment, goal formulation, crisis intervention, and facilitating seamless transitions between various services. We emphasize teamwork, conflict resolution, and the importance of fostering strong relationships with colleagues and partners.

Qualifications & Experience

  • A minimum of 2 years of experience in case management within substance use disorder treatment programs or behavioral health settings.
  • A bachelor's degree in a relevant field, complemented by some case management experience.
  • Certification or registration by pertinent authorities or relevant internship experience.
  • Familiarity with peer support techniques and confidentiality protocols.
  • Strong counseling skills and possession of a valid driver's license.
  • Bilingual abilities are advantageous.

Compensation & Benefits

The salary for this position ranges from $25.00 to $28.00 per hour, depending on your skills, qualifications, and experience.
