Position Summary:
As an ECM Care Coordinator, you will be instrumental in providing holistic care management services and facilitating community-based care coordination aimed at bridging service gaps for Medi-Cal beneficiaries, while aligning with Central Coast Alliance for Health (CCAH) performance standards. Key Responsibilities:

Direct Care and Case Management
  1. Deliver services to all participants with cultural competence, ensuring inclusivity regardless of race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, or disability.
  2. Engage and enroll eligible participants through documented outreach efforts.
  3. Conduct comprehensive assessments for individuals referred to the ECM program to identify their medical, behavioral health, and social needs.
  4. Evaluate client needs, educate on self-management strategies, and address barriers to accessing care.
  5. Monitor participant progress towards care plan objectives, providing continuous support and guidance.
  6. Inform participants of their rights, responsibilities, and the grievance procedures of the organization, ensuring services respect their freedom of choice.
  7. Collaborate with interdisciplinary team members to coordinate care and enhance outcomes.
  8. Uphold ethical and professional standards, maintaining client confidentiality and honoring cultural diversity.
  9. Provide health and mental health information to participants, families, and the community, focusing on prevention and early intervention.
  10. Ensure the safety and support of participants with physical challenges.
  11. Transport participants to appointments and community outings as necessary.
Team Collaboration
  1. Foster cooperative and professional relationships within the team.
  2. Achieve program outcomes in partnership with CCAH.
  3. Engage in regular team meetings and training sessions to enhance care coordination and outcomes.
  4. Provide feedback to management on program quality and assist in evaluating program effectiveness and participant satisfaction.
Documentation and Reporting
  1. Report any suspected abuse of dependent adults and elders to the appropriate authorities.
  2. Maintain clear, accurate, and professional documentation.
  3. Meet caseload expectations and collaborate to fulfill program requirements.
  4. Submit timely and precise invoices, billing data, and requested reports.
Qualifications:

Education and Experience:
  1. Bachelor's degree in psychology, social work, or a related field, or
  2. Three years of experience in an outreach program providing direct services to individuals facing housing challenges.
Knowledge and Skills:
  1. Experience in housing navigation, community outreach, and participant supervision.
  2. Familiarity with HIPAA, OSHA, and relevant state and local regulations.
  3. Understanding of trauma-informed care principles.
Language Proficiency:
  1. Ability to read, write, and communicate effectively in English to follow and convey care regulations and requirements.
  2. Capable of presenting information clearly and responding to inquiries.
Mathematical Competence:
  1. Ability to perform calculations related to discounts, proportions, and percentages as required by the role.
Reasoning Skills:
  1. Ability to follow written, oral, or diagrammed instructions.
  2. Competence in addressing problems involving multiple variables in various situations.
Additional Skills:
  1. Ability to complete tasks promptly, meet deadlines, and coordinate multiple activities while adapting to changing priorities.
  2. Maintain professional working relationships.
  3. Proficient in using email, word processing, and necessary software applications.
Additional Requirements:
  1. Must be at least 21 years old and eligible for the organization's vehicle insurance.
  2. Must pass background checks conducted by the Department of Justice and County.
  3. Participation in company committees may be required.
  4. Completion of all annual training as mandated by the organization.
  5. Attendance at required meetings and training sessions is essential.
  6. Adhere to established safety procedures while performing job functions.
Physical Requirements:
To successfully perform essential job functions, the following physical demands must be met:
  1. Regularly required to talk, hear, sit, and stand for extended periods.
  2. Frequently required to reach with hands and arms, stand, walk, stoop, and occasionally kneel, crouch, crawl, or lift up to 25 pounds.
