Position Summary:
As an integral part of the Housing Support Team, the ECM Case Manager is tasked with providing holistic care management services and facilitating community-based care coordination to bridge service gaps for Medi-Cal recipients, ensuring alignment with Central Coast Alliance for Health (CCAH) performance metrics. Key Responsibilities:

Direct Care and Case Management
  1. Deliver services to all participants in a culturally sensitive manner, regardless of race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, or disability.
  2. Engage and enroll eligible participants through outreach efforts.
  3. Conduct thorough assessments for individuals referred to the ECM program to identify their medical, behavioral health, and social needs.
  4. Evaluate client needs, provide education on self-management strategies, and address obstacles to care.
  5. Monitor participant progress towards care plan objectives, offering continuous support and guidance.
  6. Inform participants of their rights, responsibilities, and the company's grievance procedures, ensuring services respect their freedom of choice.
  7. Collaborate with interdisciplinary team members to coordinate care and enhance outcomes.
  8. Uphold ethical and professional standards, including maintaining client confidentiality and honoring cultural diversity.
  9. Provide information on health and mental health issues, including prevention, early detection, and remedial care to participants, families, and the community.
  10. Ensure the safety and guidance of participants with physical challenges.
  11. Transport participants to appointments and outings as necessary.
Team Collaboration
  1. Foster cooperative and professional relationships and communication.
  2. Achieve program outcome expectations in partnership with CCAH.
  3. Engage in regular team meetings and training to enhance care coordination and outcomes.
  4. Provide feedback to management for program quality improvement and assist in monitoring program effectiveness and participant satisfaction.
Documentation and Reporting
  1. Report any suspected abuse of dependent adults and elders to the appropriate authorities.
  2. Maintain clear, accurate, and professional records.
  3. Meet caseload expectations and collaborate to fulfill program requirements.
  4. Submit timely and accurate invoices, billing data, and other necessary reports.
Qualifications:

Required Skills and Experience:

Education
  1. Bachelor's degree in psychology, social work, or a related field; or
  2. Three years of experience in an outreach program providing direct services to individuals facing housing challenges.
Knowledge
  1. Experience in housing navigation, community outreach, and participant supervision.
  2. Familiarity with HIPAA, OSHA, and applicable state and local laws and regulations.
  3. Understanding of trauma-informed care principles.
Language Proficiency
  1. Ability to read, speak, write, and understand English proficiently to communicate care regulations and requirements.
  2. Capable of effectively presenting information and responding to inquiries.
Mathematical Skills
  1. Ability to calculate figures and amounts related to the position's requirements.
Reasoning Skills
  1. Ability to follow written, oral, or diagrammed instructions.
  2. Competence in addressing problems involving multiple variables in various situations.
Additional Skills
  1. Ability to complete tasks promptly, meet deadlines, and manage multiple activities effectively.
  2. Strong interpersonal skills to maintain professional working relationships.
  3. Proficient computer skills for email, word processing, and necessary software.
Additional Requirements
  1. Must be at least 21 years old and eligible for the company's vehicle insurance.
  2. Must pass background checks by the Department of Justice and County.
  3. Participation in company committees may be required.
  4. Completion of all mandated training and meetings as per company policies.
  5. Adherence to established safety procedures while performing job functions.
Physical Requirements:
To successfully perform essential job functions, the following physical demands must be met:
  1. Regularly required to talk, hear, and sit or stand for extended periods.
  2. Frequently required to reach with hands and arms, stand, walk, stoop, and occasionally kneel, crouch, crawl, or lift up to 25 pounds.
  3. Must be physically, mentally, and occupationally capable of performing assigned tasks.
