POSITION OVERVIEW
As a vital member of our healthcare team, the Licensed Vocational Nurse (LVN) plays a crucial role in the Resident care planning process, particularly focusing on the MDS and MMQ protocols.
KEY RESPONSIBILITIES
The following duties are essential to the role, and additional responsibilities may be assigned as necessary:

Assist the Resident Assessment Coordinator in fulfilling their duties, as needed.
·
Conduct thorough assessments of Residents to inform and direct their individualized care plans.
·
Facilitate orientation sessions regarding the MDS process for all relevant staff members.
·
May oversee the operations and educational aspects of information systems within the clinical department.
·
Responsible for liaising with the Medicare benefits hotline in accordance with company protocols.
·
Create and maintain a schedule for periodic assessments to ensure compliance and timeliness.
·
In the absence of the Resident Assessment Coordinator, may assume their responsibilities under the supervision of a Registered Nurse (if applicable).
Ensure all required documentation is completed in line with company policies and applicable state and federal regulations.
QUALIFICATIONS
·
Must be a graduate of an accredited nursing program.
·
Possess a valid State license as an LPN/LVN.
·
Familiarity with OBRA, the Perspective Payment System, and the MDS process is essential.
