End-of-Life Care Coordinator

We are seeking an experienced Hospice RN to join our team at Stillwater Hospice. This role offers the opportunity to make a meaningful difference in the lives of patients and their families, while working collaboratively with a compassionate and dedicated team of healthcare professionals.
Key Responsibilities- Assess patient needs and develop individualized care plans, incorporating input from interdisciplinary teams and patient preferences.
- Coordinate patient care through effective communication with healthcare providers, families, and caregivers, ensuring seamless transitions and smooth care delivery.
- Provide patient and family-centered care, addressing physical, emotional, and social requirements through education, counseling, and support.
- Maintain accurate and detailed records of patient progress, collaborating with healthcare teams to ensure timely and effective interventions.
- Bachelor's degree in nursing, with one to two years of recent acute care experience and one to two years of recent experience in home care.
- Current licensure in state, with a valid driver's license and reliable vehicle for travel to patient homes.
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Able to lift, position, and transfer patients, as well as handle heavy lifting and bending, with consideration given to visual acuity and hearing to perform required nursing skills.
