Job Description:

The Support Coordinator / Service Coordinator / Case Manager plays a critical role in developing individualized care management plans, implementing services, and monitoring outcomes to ensure high-quality, cost-effective care. This position assists clients in identifying needs and communicating requests, which may include housing, medical, social, vocational, and other community services.

Essential Duties and Responsibilities:

* Conduct assessments to identify participant needs and desires for service delivery
* Develop Person-Centered Service Plans and modify as needed
* Ensure participant choice of providers by providing informed decision-making opportunities
* Coordinate with other essential individuals and entities to ensure seamless care delivery
* Research alternative solutions to meet participants' needs
* Maintain confidentiality and adhere to HIPAA guidelines
* Attend required trainings and recertification classes
* Collaborate with supervisor/director(s) to understand participant needs and abilities
* Travel up to 50% as needed

Qualifications:

* BS/BA degree in related coursework
* Minimum 3 years experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities
* Experience with Microsoft Office and state-maintained databases
* Must maintain proficiency in company-sponsored training and certifications
* Valid Driver's License and personal vehicle insurance coverage for liability and current vehicle registration
