Job Title: Healthcare Coordinator - Care Manager

**Job Summary:**
We are seeking a skilled Care Manager to join our team at Great Plains Tribal Leaders Health Board Inc. As a Care Manager, you will play a vital role in providing high-quality patient care and promoting seamless coordination across the healthcare continuum.

Responsibilities:
• Obtain initial patient screening history, perform visual assessments, obtain vital signs, review health reminders and immunization status, and conduct physical assessments.
• Maintain awareness of patient comfort and safety needs, explain procedures and treatments to gain cooperation and understanding, and alleviate apprehension.
• Assist medical providers during treatment and examination of patients as requested.
• Screen patients for health problems, assess clinical conditions, recognize serious situations, inform medical providers, and take appropriate preplanned measures.
• Operate and monitor various medical equipment, including suction apparatus, nasogastric suction, defibrillator machines, scales, vital sign machines, IV pumps, and EKG machines.
• Administer prescribed medications, assess patient responses, and document assessment and medication administration.
• Provide oxygen therapy, document patient responses, and take appropriate action when oxygen therapy is ineffective or has limited effectiveness.
• Maintain accountability for delivery of quality nursing care, communicate with medical providers and supervisors, and coordinate patient needs.
• Collect specimens, order laboratory and x-ray studies as authorized by medical providers, and identify high-risk complications.
• Act as an advocate for patients' health care needs, understand methods for assessing patients' physical and mental impairments, assist individuals with developing health goals, and understand psychological characteristics of wellness.
• Manage the high-risk patient registry, oversee systems for identifying high-risk patients, ensure validity of the registry, and collaborate with Information Technology on registry functionality.
• Develop and maintain tracking systems for patient care coordination and care management across the continuum, including care transitions, primary, and specialty care, tracking referred patients for admission to other healthcare facilities, and performing chart reviews to determine timeliness of requested services.
• Provide technical assistance to staff for clinical issues related to assigned patients, respond to insurance providers and third-party agencies to ensure maximum reimbursement, implement and monitor the CHEF program for the CHS department, and assure timely documentation for reimbursement.
• Conduct comprehensive assessments of patients' physical, mental, and psychosocial needs, collect indepth information about patients' situations and functioning, develop comprehensive care management plans that address identified needs, and utilize behavioral strategies to help patients adopt healthy behaviors and improve self-care in chronic disease management.
• Assist patients in navigating the healthcare system, coordinate specialty care, follow up on test results, and execute specific care management activities and/or interventions that lead to accomplishing goals set forth in care management plans.
• Partner with external care management programs to coordinate care, provide ongoing evaluation and documentation of patient progress/risk status, and communicate with care teams.
• Gather sufficient information from relevant sources about case management plans and their activities/services to enable case managers to determine the effectiveness of plans.
• Record accurate, timely, and appropriate information in patients' electronic health records, document observations, nursing interventions, therapeutic measures administered, and status of coordinated activities between nursing and other professional disciplines.
• Initiate patient education plans according to individualized patient needs, as prescribed by medical providers and/or OHC policy, including patient and family instruction, notify patients of lab results, and provide advice to patients per provider instruction.
• Assist with referrals and/or follow-up care based on results of laboratory and radiology reports, and the mental health and well-being of patients as recommended by medical providers.
• Respond to life-saving situations based upon nursing standards, policies, code procedures, and established protocols, participate in quality improvement activities such as GPRA, PI, IPC, nursing audits/peer review, infection control, case management, and nursing care conferences, and comply with federal laws and regulations as required by the Health Insurance Portability and Accountability Act (HIPAA).

Requirements:
