Cardiology Care Coordinator

We are seeking an experienced Registered Nurse to join our team as a Patient Nurse Navigator Cardiology at Next Move Healthcare.
As a Patient Nurse Navigator Cardiology, you will specialize in guiding patients and their families through the complexities of cardiology care.
Your key responsibilities will include:
- Primary Contact: Acting as a primary contact for patients and families, guiding them through the healthcare system.
- Care Coordination: Coordinating and improving the efficiency of care, ensuring positive patient outcomes.
- Patient Education: Educating patients and families about treatment options and healthcare resources.
- Care Program Development: Assisting in the development and implementation of patient care programs.
- Data Analysis: Analyzing patient data to improve care quality and close healthcare gaps.
- Collaboration: Collaborating with healthcare professionals to align resources with patient needs.
- Education and Outreach: Supporting the development of educational and outreach programs.
To be successful in this role, you will need:
- Nursing Education: Must be a graduate from an accredited nursing program.
- Licensure: Holds a current RN license from the South Dakota Board of Nursing.
- Certification: BLS Certification from the American Heart Association.
- Preferred Qualifications: Preferred Bachelor's degree in Nursing and 1+ years of Case Management Experience or 1+ years of Patient Care Experience.
