Job Title: Care Coordinator Specialist

Salary: $65,000 - $80,000 per year

About the Job

We are seeking a highly motivated and dedicated Care Coordinator Specialist to join our team at Great Plains Tribal Leaders Health Board Inc. The successful candidate will be responsible for implementing an Evidence-Based Curriculum for parents who are pregnant or parenting a child 0-5 years of age.

Key Responsibilities
  • Implement Evidence-Based Curriculum for parents during home visits on early childhood development, parenting education, and community resources.
  • Conduct family-centered assessments and goal planning with families, connect them to outside resources and referrals, and provide coordination and advocacy for services needed to assist families in working towards their goals.
  • Manage a caseload of 15-20 clients and their families, ensuring project objectives and deliverables are met and comply with GPTLHB and GP-TMIECHV policies and procedures.
Requirements
  • Completion of a formal training program or an associate's degree and one (1) year of relevant experience; or a high school diploma or GED and three (3) years of relevant experience.
  • Familiarity and/or experience working with American Indian populations and respect for and knowledge of traditional, cultural, and spiritual practices of diverse American Indian communities.
