Responsibilities:

  • Assess patient needs and develop individualized care plans that address physical, emotional, and spiritual concerns.
  • Implement evidence-based interventions to manage pain, symptoms, and other health issues.
  • Collaborate with interdisciplinary teams to ensure comprehensive care coordination and effective communication.

Requirements:

  • Current RN licensure in the state and at least 1-2 years of experience in hospice care or home healthcare.
  • Ability to lift 25 pounds, stand for long periods, and travel to patient locations as needed.
