Position Overview: The Patient Care Coordinator Assistant - LPN plays a vital role in the orchestration of patient care and associated services.

This position enhances the quality of care through a collaborative approach that coordinates, monitors, and assesses services based on the individual needs and interactions with patients.

This is done in compliance with all relevant laws, regulations, and the standards set forth by Life Care.
Qualifications:
  • Nursing diploma (associate's or bachelor's degree in nursing)
  • Minimum of one (1) year of clinical experience in a post-acute care environment is preferred
  • Experience in case management, utilization review, and discharge planning is advantageous
Key Skills:
Excellent organizational abilities and proficiency in prioritizing and managing tasks and responsibilities.

  • In-depth knowledge in the field of nursing practice
  • Familiarity with practices, procedures, and the laws, regulations, and guidelines governing operations in a post-acute care facility
  • Ability to implement and interpret departmental programs, goals, objectives, policies, and procedures
  • Competency in all areas of responsibility, including but not limited to: managerial duties, patient rights, and safety protocols
  • Maintain confidentiality of all proprietary and sensitive information
  • Adhere to company policies, including those related to harassment and compliance
  • Exhibit integrity and professionalism by following Life Care's Code of Conduct and completing necessary compliance training
Technical Proficiency:
Demonstrate effective use of relevant computer systems, including the ability to input and retrieve data efficiently.

  • Act as a liaison between external case managers, families, physicians, and community resources
  • Provide training and education to patients, families, associates, and other care providers
  • Implement standards of practice for care management, ethical conduct, and functions pertinent to care coordination
  • Ability to read, write, speak, and comprehend the English language
