About the Role:

HonorHealth is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will collaborate with primary physicians and other healthcare team members to develop patient-centered goals and actions. Your primary responsibility will be to ensure the formulation of a realistic and definitive goal that represents the total care needs and resources of the patient/client and family.

Key Responsibilities:

  • Coach and guide patients and their caregivers in addressing critical issues and self-management tasks to assist patients in their recovery efforts.
  • Coordinate and review patients' progress and ability to manage healthcare issues in conjunction with clinical practitioners.
  • Assist patients with medication reconciliation, coordination of post-discharge home care needs, self-management support, follow-up care, supportive care, advanced care planning, community resources, and long-term planning needs.
  • Ensure safe handoff of care for hospitalized/post-acute care patients from Transitional Care Managers to Care Coordinators.
  • Maintain and manage caseloads working independently with a minimum of direction, anticipate and organize workflow, work with a high volume caseload, and deal with rapidly changing priorities.

Requirements:

  • High School Diploma or GED Required
  • 1 year outpatient, ambulatory care, specialty care, or community healthcare experience Required
  • Basic Life Support (BLS) CPR Card Required
  • Fingerprint Clearance Card - State Current State of Arizona Finger Print Clearance Card within 8 weeks from date of hire for those employees who conduct home visits. Required
