Ackerman Cancer Center, a leading entity in cancer care, is on the lookout for a committed and proficient part-time Oncology Case Manager. As the only physician-owned proton therapy center globally, we prioritize innovative and patient-focused treatment.
Role Summary
The Oncology Case Manager at Ackerman Cancer Center plays a crucial role in orchestrating and overseeing the care of oncology patients throughout their treatment journey and beyond at our various locations. This position requires close collaboration with physicians, nursing staff, and other healthcare professionals to ensure that patient needs are addressed promptly and compassionately. Additionally, the Oncology Case Manager will provide essential education and support to patients, their families, and other healthcare providers throughout the cancer care continuum.
Key Responsibilities
- Collaborate with the Oncology Social Worker to facilitate patient navigation, including referrals to relevant community resources that offer quality information, financial aid, and psychosocial support.
- Assist patients with application processes for programs such as Medicaid, ACC Foundation, and other funding sources for care.
- Oversee transportation and accommodation arrangements for patients.
- Manage dental clearance and home health service requests.
- Ensure timely processing of SSDI requests.
- Prepare agendas, discussion points, and evaluations for weekly patient care meetings.
- Work alongside the Oncology Social Worker to identify patient support program needs and refer patients to ACC initiatives.
- Support the organization and planning of patient support groups and team meetings.
- Coordinate with the Oncology Social Worker and clinic team regarding nutrition assistance programs.
- Partner with the Community Relations team to organize patient events.
Qualifications
Essential
- Bachelor's Degree in Psychology, Sociology, Social Work, or a related field.
- Excellent interpersonal skills with effective communication abilities.
- Strong organizational skills and attention to detail.
- Proficiency in MS Office applications.
- Able to manage multiple tasks and diverse responsibilities efficiently.
Work Schedule: Part-time; 20-25 hours per week.
