Ackerman Cancer Center, a leader in innovative cancer care, is in search of a compassionate and skilled part-time Oncology Case Manager. As the only physician-owned proton therapy center globally, we are dedicated to delivering exceptional, patient-focused services.

Role Summary

The Oncology Case Manager plays a vital role in overseeing and coordinating the care of patients undergoing cancer treatment at Ackerman Cancer Center. This position requires close collaboration with physicians, nursing staff, and other healthcare professionals to ensure that patients receive timely and empathetic support throughout their treatment journey. Additionally, the Oncology Case Manager is responsible for providing education and assistance to patients, their families, and other healthcare providers.

Key Responsibilities

  • Collaborate with the Oncology Social Worker to facilitate patient navigation, including referrals to local and national community resources that offer valuable information, financial aid, and psychosocial support.
  • Guide patients through various application processes for assistance programs, ensuring they receive the necessary support.
  • Oversee transportation and accommodation arrangements for patients.
  • Coordinate dental clearances and home health service requests.
  • Ensure timely processing of Social Security Disability Insurance (SSDI) requests.
  • Prepare agendas and evaluation reports for weekly patient care discussions.
  • Work alongside the Oncology Social Worker to identify and implement patient support initiatives.
  • Assist in organizing patient support groups and team meetings.
  • Collaborate with the clinic team regarding nutrition assistance programs.
  • Partner with the Community Relations team to plan events for patients.

Qualifications

Essential

  • Bachelor's Degree in Psychology, Sociology, Social Work, or a related field.
  • Excellent interpersonal skills with effective communication abilities.
  • Strong organizational skills and attention to detail.
  • Proficiency in Microsoft Office Suite.
  • Capability to manage multiple tasks and diverse responsibilities.

Work Schedule: Part-time; 20-25 hours per week.
